If you require the rehabilitation support of a physiotherapist for a chronic or routine issue, this is a locality service that you are able to self refer to through community physiotherapy. Community physiotherapy self referral forms are available to download from this website or alternatively are available to collect from one of our team at reception.
An Advanced Physiotherapy Practitioner (APP) now works directly in your GP practice. They are experts in diagnosing, investigating and managing muscle, bone and joint problems such as back, neck, shoulder or knee pain. These are known as musculoskeletal (MSK) problems. If you are over 16 years and think you have an MSK problem you can book an appointment with them rather than your GP.
The APP will assess you and talk to you about management options. This may include advice on how to manage the problem and exercises for you to work on at home. If further tests, investigations or rehabilitation are needed they can organise this. The APP works very closely with your GP and other colleagues in the practice to ensure best holistic care.
You can ask reception for an appointment with the APP or your GP may suggest this as the best option.
